Abstract
Fairness in sport competition is often dictated through law and policy. The development of policy arises from not only an understanding of physiology and the potential consequences that can result from unfair competitive practices, but also the desire to be culturally and socially sensitive. This manuscript provides a brief historical review of legal policies concerning transgender sport participation and examines the physiological evidence and current practices for fair competition. Critical to the specific issue of transgender participation in sport is the distinction between gender and sex.
