During my faculty development leave, I wrote and assembled a first of its kind manual devoted to clinical assessment in music therapy (published by the American Music Therapy Association):

(From the back cover) "Clinical assessment, whether occurring at the beginning, during, or near the conclusion of treatment, is a cornerstone for effective music therapy intervention. It is surprising, therefore, that few music therapy texts are dedicated solely (or in large part) to clinical assessment. The Music Therapy Assessment Manual: A Guide to Data-Based Decision Making is intended to help fill a gap in the music therapy literature for music therapy students, interns, entry-level professionals, and experienced clinicians looking to refresh their assessment skills.

The manual focuses on assessment with an emphasis on how it plays a role during all stages of the treatment process. Further, the manual provides guidelines on how to conduct an assessment by linking referral, initial assessment, treatment planning, progress monitoring, treatment evaluation, and termination. Finally, the manual positions the practice of assessment as a music therapist's professional responsibility by aligning it with current standards and structures."
