Introduction/Context/Diagnosis
This case is about the dental management of a 78-year-old male patient with significant systemic health issues, including hypertension, hyperlipidemia, and polycythemia vera, under treatment with valsartan and atorvastatin. The patient expressed a desire to improve his dental aesthetics, particularly through the application of veneers. Clinical examination revealed an edentulous space at the locations corresponding to teeth #19 and #30, with #30 fractured at the gumline, and incisal chipping of the lower anterior teeth. The treatment plan was multifaceted, prioritizing the stabilization of the patient's periodontal health through prophylactic cleaning, followed by caries control. This involved resin fillings on teeth #4 (disto-occlusal) and #21 (disto-occlusal). Tooth #30 was assessed as non-restorable and asymptomatic, leading to its extraction. To address the resulting edentulous spaces and to stabilize the occlusion, bilateral zirconia bridges were fabricated and delivered, replacing the lower first molars. Additionally, the lower anterior teeth (#23, #24, #25, #26) underwent resin composite filling treatments to correct incisal chipping, enhancing structural integrity and aesthetics. Following these restorative interventions, the patient’s occlusion was evaluated and found to be stable. Combined with his low to medium smile line and age considerations, he was deemed an excellent candidate for Emax veneers. These veneers were chosen for their esthetic qualities and durability, suitable for the geriatric patient. This case highlights the importance of a comprehensive approach in treating elderly patients with complex medical histories. It emphasizes the necessity of integrating systemic health management with functional and esthetic dental restoration, ensuring that both health and quality of life are enhanced through tailored dental care.
