Abstract
Stroke recovery extends beyond hospital discharge, yet community-dwelling stroke survivors and their informal caregivers often have limited access to continued education, wellness resources, and community-based support.
This gap contributes to caregiver burden, reduced occupational participation, and decreased quality of life.
